Referrals
To enquire about care at Meadow Court, please contact us here, or via phone or email. Our experienced team will guide you through our process, which includes an assessment to ensure we can provide the best possible care and support.
Referrals - How our process works:
Step 1
Call or email us with some brief details regarding your referral.
If we feel that Meadow Court could be an appropriate provider for the individual, we will request a more detailed overview. This is usually in the format or a recent and current Care Act Assessment.
Step 2
We will seek to arrange an assessment to spend time with the person along with the people involved with their current care arrangements; this can include social worker, care provider, and their family. This enables us to better understand their current situation, support needs and ambitions.
Step 3
Once we have built our recommendation for support, produced a funding proposal detailing the support hours required, this will be returned back to the social worker, sourcing care team, or commissioners for approval
Step 4
Once funding has been agreed and where appropriate we will invite the adult to our site with their family to meet the team and ask any questions that they may have.
Step 5
We will agree on a move in date and start the transition process. This can be an exciting time for some but for other this can be met with trepidation, so we will be on hand to offer support and any reassurance throughout the transition process.
